Before we can confirm your Story Session, we'll need you to sign a release form and confidentiality agreement. By signing, you consent to having your story recorded and your image used for the purposes of this book. Additionally, by signing the confidentiality agreement, you agree not to discuss the details of the project publicly until after its publication.
